The Excel V (Cutera, Brisbane, CA) is busy all day long treating a full spectrum of vascular and benign pigmented conditions,’ explain Drs Joely Kaufman and Jeremy Green, of the Coral Gables office of Dr Brandt Dermatology Associates, where they both practice.

‘The Cutera Excel V is the most used vascular device in our practice, effective for nearly all of my cosmetic patients. Dr Green explains, ‘The Cutera Excel V Laser is a combination of two wavelengths: 532 nm green light KTP laser and 1064 nm infrared Nd:Yag laser. These two wavelengths allow me to provide a customized treatment for any vascular lesion, from superficial to deep. With Excel V’s high-energy 532 nm laser, I can effectively treat small telangiectasias, cherry angiomas, pink–red port-wine stains and diffuse erythema. The long-pulsed 1064 nm laser is ideal for deeper or thicker vascular lesions, including leg veins, periorbital blue veins, venous lakes, and nodular port-wine stains.’

The exclusive high-powered 532 KTP provides a no-downtime procedure for reds and browns in a single treatment.

‘I used to tell patients that it was difficult to treat vascular lesions and that it would take up to three sessions before they would see a major improvement. Now, with the Excel V I can tell patients that after just one session they will see a dramatic improvement in their vessels,’ said Dr Kaufman.

‘I use the Excel V for multiple types of lesions. I love it for pigmented lesions, for vascular lesions both on and off the face. In Miami, with all the sun damage that we have here, I’ve always had trouble treating people who were tan with poikiloderma of the neck and chest. I used to rotate lasers; we would try several different treatments and still there’d be very little improvement. Now, with the Excel V, we are seeing excellent results for poikiloderma in just one or two treatments.’

‘The first thing you notice about the Excel V is the ergonomic handpiece, with a cool sapphire window that allows for patient comfort. It affords perfect visibility through the window of the target lesion, and this allows me to tailor each treatment with my fluence, pulse width, and spot size, to precisely get the response I’m looking for. I am able to see the light–tissue interaction so easily through that window and I can watch the vessel collapse,’ said
Dr Kaufman.

Other than its versatility, Excel V offers variable continuous contact cooling through a sapphire window that allows the temperature to be precisely adjusted from 5°C to 20°C.

‘You can cool before, during and after the laser pulse to protect the skin’s surface,’ said Dr Green. ‘You don’t have to bother with cryogen or other consumables. Once you purchase the laser, there are no more costs.’

The Excel V also gives the practitioner unparalleled target visualisation. ‘Instead of requiring you to lift the handpiece to see results, the Excel V lets you see the laser–tissue interaction as you treat,’ said Dr Green. ‘It’s very gratifying. I encourage family or friends in the room to watch as I am treating, and it creates one of those ‘wow’ moments as they see stubborn vessels disappear.’

Laser Genesis

The microsecond 1064 nm treatment — known as Laser Genesis — is used for non-ablative skin revitalization, improving diffuse redness, dyspigmentation, and stimulating collagen.

Dr Green explains, ‘One of the most popular treatments at our office is what we call a combo treatment. We start with the Excel V 532 nm treatment for blood vessels on the face, for rosacea for example, and then we follow it with the full-face Laser Genesis — what we call the laser toning procedure — and patients absolutely love it. Again, we have a whole cohort of patients that come every 4–6 weeks specifically for this treatment. They love it because when they leave the office, they feel like they’re glowing and there really is no downtime.’
