The Pellevé Wrinkle Reduction System precisely delivers energy to the dermal tissue using advanced radiofrequency (RF) technology to induce collagen contraction without damaging the epidermis. Pellevé’s unique RF technology emits energy in the form of electromagnetic waves, which creates a gradual heated treatment zone. Increased resistance causes heat build-up at the subdermal junction. Heat induces collagen denaturation and collagen contraction. Protein stimulation causes new collagen formation and production in the dermis, which also results in skin tightening. The result is a noticeable improvement in skin quality, firmness and texture.

When did you first start offering Pellevé to your patients?

Since May of 2012, when we introduced the Pellevé System, we have carried out around 150 cases, or approximately 15–20 cases per month. It has been a very popular procedure in our clinics.

What has been the response and feedback from your patients?

Most patients are satisfied because there are no side-effects after treatment and they feel comfortable. Some patients experience dramatic changes in their skin texture and wrinkle improvement. Usually patients feel a smoothness in skin texture and an immediate skin lifting. Fine wrinkles of the forehead and upper cheeks are also improved. Periorbital wrinkles can be improved after repeated treatments, usually a series of three sessions. We have also seen a skin brightening effect that patients appreciate.

Who are the ideal candidates for Pellevé?

In our clinics, we recommend Pellevé for women aged 30–70 years, and women who have full, plump cheeks. The ideal patients are those with moderate skin laxity, very thin skin, pain-sensitive, those who have dilated pores, and patients who desire a skin lifting treatment. We also recommend Pellevé treatments for women who are preparing for their wedding, a big party, or other ceremony as a single treatment or if there is time, a package of treatments. We see many television stars in our clinic who have been treated with Pellevé. They have said that a single treatment is also good before they have to be on camera or go to a photoshoot because there is an instant effect.

What advantages does Pellevé offer over other skin tightening or RF systems?

Pellevé is not painful, and there are no side-effects, or spark and burn during the treatment. It is not only good for wrinkle reduction, but also delivers skin tone improvement. Patients feel very comfortable and relaxed during the procedure, which is also very important, so they will come back for additional procedures.

Can you describe the challenges of treating Asian skin types?

There is much diversity among Asian skin tones that can range from very light, pale skin, to light or dark brown skin. Pigmentation issues are a major concern as possible side-effects of dermatologic conditions and skin treatments. The potential for post-inflammatory hyperpigmentation (PIH) often presents a challenge. For example, some laser- and light-based procedures can cause melasma to worsen if it affects the pigmented cells, and leaves white spots in the treated areas. Many Asian patients do not realise the importance of good sun protection to prevent premature skin ageing, maintain skin tone, and minimise uneven skin and pigmentation problems.

Is Pellevé a safe treatment for skin of colour?

We explain to patients that Pellevé treatments are absolutely safe for Asian skin types, especially dark skin and melasma patients, which are very common problems. We can also offer Pellevé during any season, even for patients who go out in the sun a lot, because there is no response in melanin, and therefore no risk of pigment changes from the treatment.

How do you combine Pellevé with other treatments like toxins, fillers, or skincare?

We routinely combine Pellevé with other lasers, toxins, fillers, and skincare. This is another nice advantage of the system, which is ideal for combination therapies that some patients need. For example, we use Pellevé with Laser Toning for melasma patients. For dilated pores, we may suggest Pellevé with fractionated laser therapy. For treating crow’s feet, we can offer the patient Pellevé with BOTOX®. For deep nasolabial folds, we will also combine dermal fillers with a Pellevé treatment. We use Pellevé with skincare treatments as well, such as iontophoresis, superficial peelings, and topical agents. We frequently combine Pellevé with every procedure offered in our clinics.

Please explain how Pellevé has had a positive impact on your clinics.

Pellevé has been an excellent adjunct to all skin rejuvenation procedures, and has led to more treatments for each patient. The reasonable cost of the device plus minimal disposable costs make it a very affordable treatment to offer in our clinics. For patients who are particularly worried about pain, scarring and other side-effects, Pellevé is a good solution. We can give them something that will offer improvement in the tone and texture of their skin, without discomfort, redness or a high risk of pigment changes. Before Pellevé, we did not have much to offer these patients, so it has enabled us to treat a new group of patients in our clinics safely and effectively.
